LillyDirect is Eli Lilly's answer to the question 'why can't I just buy the real drug?' — the manufacturer dispensing its own medicines, with the most complete published self-pay price ladder in this review: every Zepbound dose from $299 (2.5 mg starter vials and KwikPens) through $449 monthly with refills inside 45 days, against regular prices up to $699. It is also the only entry here with publicly documented FDA enforcement history: a September 2025 warning letter found a promotional video touting the self-pay channel was false or misleading and omitted all risk information. That letter concerns marketing, not the products — which remain the genuine article, dispensed by Lilly's own Indianapolis pharmacy.
