How Metformin works
- Mechanism
- Reduces hepatic glucose production and improves insulin sensitivity. Weight effect is modest and secondary to the glycemic effect.
- Administration
- Oral, daily, usually with an extended-release formulation to improve tolerability.

- Gradual escalation over several weeks, primarily to manage gastrointestinal tolerability.

- Modest weight reduction — typically low single digits as a percentage — alongside meaningful glycemic improvement in appropriate patients.

Known side effects
- Gastrointestinal effects, substantially reduced by extended-release formulations

What Metformin means for your budget
A useful test of vendor honesty
Every vendor lists metformin. Far fewer can show you what share of their population is maintained on it without escalating. Ask for that number — it separates vendors with a step-therapy protocol from vendors with a step-therapy slide.
It does not compete with GLP-1s on weight
Metformin is not an alternative to incretin therapy for a member who needs substantial reduction. Its role is identifying the members who never needed the expensive drug, not substituting for it in those who do.
