How Semaglutide works

Mechanism
Mimics endogenous GLP-1: slows gastric emptying, increases satiety signaling, and stimulates glucose-dependent insulin secretion. The appetite effect, not the glycemic effect, drives the weight outcome.
Administration
Once-weekly subcutaneous injection. An oral daily formulation exists but is indicated for type 2 diabetes rather than weight management.
Titration
Typically initiated at a low weekly dose and escalated in steps roughly every four weeks over four to five months to the maintenance dose. That escalation window is where most discontinuation happens.
Typical result
In registrational trials, average body-weight reduction in the mid-teens as a percentage at roughly 68 weeks. Real-world results run materially lower, driven almost entirely by discontinuation rather than by non-response.
